Background: A systematic, all-dimensional long-term care service system for dementia elderly has not been built in China till now, which has significantly impacted on the quality of life for dementia elderly and family members, carers and society. One of aspects relating to the development and implementation of dementia policies and plans is to build the essential care service package for dementia elderly. Methods: From July to October, 2018, a total of 1242 dementia elderly in five cities in China by a cluster sampling were investigated with the self-designed questionnaire about longcare service demands. We take three principles into account for establishing basic care service package for dementia elderly, which are to take person-centered care service demand as the orientation, universal care coverage on safeguarding vulnerable populations as the focus, emphasis on the feasibility, sustainability and affordability. The basic care service package for dementia elderly was built from four dimensions, that is, daily life care, rehabilitation nursing, behavior management and function maintenance, by using expert consulting of Delphi method. Results: The basic care service package for dementia elderly was composed of a series of core care service items in order to guarantee that all dementia elderly enjoy equal care services, basing on public financing and implementing strategies and taking China’s situations into account. The basic care service package of three, second and first-level included different items of care for the mild, medium and severe dementia elderly, respectively. Conclusions: The basic care service package for dementia elderly based on the demands among dementia elderly advanced in the paper was a practicable and effective quantitative management means. Some safeguard mechanisms and operational implementing pathways of basic care service package for dementia elderly in China are proposed, which are to make dementia a public health and social care priority, build the all-round raised capital mechanism under the leading investment of government, incorporate the basic care service package into the long-care nursing insurance system, strengthen the construction of propaganda and care network for dementia, construct the system of care demand assessment and classification service for dementia elderly.
